The Student Travel Registry must be completed by:
- Students traveling abroad as part of a group with a UF Medical mission or a service program, a FAB program, or any other non-credit bearing program outside the US, that is associated with UF.
Student Group Travel
All students traveling abroad as part of a group with a UF Medical mission or a service program, a FAB program, or any other non-credit bearing program outside the US, must complete the Student Travel Registry and purchase CISI health insurance through the UFIC. The cost is $38.50/month or $10.50 per week/per person up to three weeks (effective August 1st and subject to change with contract renewals) to be paid by check or cash.
Since student group travel will need to be approved by the UFIC first, the group leaders should submit the following to the UFIC:
- A complete itinerary of the proposed trip, including contact information onsite to be sent to the Susanne Hill.
- A completed CISI group enrollment spreadsheet as an Excel file.
- One check made out payable to the University of Florida that covers all trip participants at $38.50/month per person, or $10.50 per week/per person up to three weeks (effective August 1st and subject to change with contract renewals) submitted to the front study abroad desk at UFIC.

Individual StudentTravel
Students on individual international trips that have been approved by UF will need to register with the Student Travel Registry and purchase CISI health insurance through the UFIC by submitting the CISI enrollment form and the associated fee to be paid by check or cash.
